New Patient Paperwork
If you decide you want to make Dental Care at Pleasant Hill your dental health home, our staff will help you schedule your first official appointment with our dentist. Prior to this appointment, we ask that you complete our new patient paperwork. This provides our dentist with basic contact information as well as a detailed history of your dental and physical health. We ask that you complete the new patient paperwork as thoroughly as possible to ensure our dentist can provide you with the highest quality dental care. For your convenience, you can download the new patient forms from the following link and fill them out at your leisure:
Otherwise, we ask that you arrive at least 20 minutes early for your first appointment so you can complete these forms in our office.
After closely reviewing your new patient paperwork and dental health history, our dentist will conduct a thorough examination of your teeth, gums and other oral tissues to assess the current state of your oral health. Our dentist will look for signs of decay, infection and other issues. After the exam, we will perform a thorough teeth cleaning to eliminate plaque, tartar and any other buildup that could harm your teeth and gums and interfere with our x-rays.
When your exam and teeth cleaning are finished, our dentist will take x-rays of your mouth to analyze your teeth and jawbone. Our digital x-rays allow our dentist to see cavities, impacted teeth, bone loss and other dental problems deep beneath the surface of your teeth and gums. After your first visit, you can expect to have x-rays taken annually to monitor your oral health, although our dentist may use this dental imaging tool to diagnose sources of pain if you present such symptoms outside of a routine exam.
At the conclusion of your exam, cleaning and x-rays, our dentist will discuss the results with you. If any troubling issues were found, such as cavities or gum disease, our dentist will inform you of your treatment options so you can eliminate the problem and enjoy better oral health going forward. We know additional dental work can be daunting, particularly when it comes to payment, so our staff will provide you with a detailed breakdown of the cost of any recommended treatments and work with you to develop a payment plan for any portion not covered by your insurance.
If our dentist recommended additional treatments to address any issues discovered during your exam, our staff will work with you to schedule those appointments in a timely manner. If, however, no additional treatments were recommended, we will schedule you for your next biannual exam and cleaning six months from now.
